Registering with a GP Practice

You can register at the practice if you live in the area. Use our boundary map to check your .

The practice will only accept patients who live in our boundary area. The Find a GP service will help you if you fall outside our area.

Please note that if you submit registration forms  and do not live within our boundary your registration will be rejected.

If using the forms below you will need for each member of your family, to complete a GMS1 form and the document relevant to your age (please also note the information on the registration welcome pack). You will also need to bring two forms of identity with you, one of these must be a photo ID such as a passport or driving licence and the other must be a utility bill or equivalent showing the address where you are registering.

When a patient registers at a practice their medical record is electronically transferred to their new practice, who then become the patient’s registered GP and a patient should be contacting them in the first instance for medical help as they hold the most relevant and up to date information on the patient and are best place to assess and prescribe for them. This includes patients when they register at a GP practice at the request of the university that they are attending. When contact is made by a patient in these circumstances they are requested by our reception team to contact their registered GP to access care.

We require all new patients to undertake a health check with a practice nurse.

Once registered you can access the NHS APP to booking appointments, ordering repeat medication and viewing  your medical records.
