Summary Care Records (SCR) are an electronic record of important patient information, created from GP medical records. They can be seen and used by authorised staff in other areas of the health and care system involved in the patient’s direct care. You can choose not to have your personal clinical data held on the national electronic summary care record.
Your medical records
You can access your medical records through the NHS APP or Patient Access. Patients are being given online account access to their future, or prospective, full general practice health record including free text, letters and documents.
